Hybrid breast augmentation: a perfect combination of natural and beautiful aesthetics

In our experience, this technique combines the advantages of breast implants and fat transfer. The procedure achieves a harmonious and aesthetically pleasing result without major compromises.

  • Smaller implants enable (through smaller incisions) precise volume enlargement and shape correction

  • The fat transfer (approximately 100 - 150ml per breast) ensures natural softness and authenticity of breast tissue.

For patients with unevenly sized breasts, different implant sizes can be used. We exclusively use breast implants with a smoother surface as they cause less foreign body reaction (more on this under 'Risks and Potential Complications’). The filling material for all our implants is a cross-linked silicone gel ('high cohesive gel') that is form-stable yet soft.

Round implants add volume to the entire breast, including the area above the nipple. They can create a beautifully shaped décolleté.

Because the implants have a round base, the shape of the breast would not change if the implants were to rotate (unlike anatomical implants)

And therefore no correction would be necessary. Depending on the thickness of the implants (also called 'profile' or 'projection'), the naturalness of round implants may be perceived as lower, especially from a side view.

Anatomical implants are teardrop-shaped. They create a more natural contour and breast shape, especially in the lower part of the breast. Teardrop implants are suitable for women who prefer a slightly sloping shape with less volume. There is a risk of visible rotation of the implant, which could require correction surgery.

(Motiva Ergonomix) combine the advantages of both round and anatomical implants. The implants have a round base. For this reason, the implants are allowed to rotate because the shape of the breast would remain the same. It is important to realize that the silicone gel of Ergonomix implants is very soft and flexible.

 

Therefore, these implants tend to form a more teardrop-shaped breast contour when the body is upright.

Breast Augmentation by Fat Transfer?

Fat transfer for breast augmentation, also known as lipofilling or autologous fat transplantation, is an innovative method for breast augmentation that uses the patient's own adipose tissue. With autologous fat transplantation, a very natural and harmonious breast augmentation can be achieved.

How does breast augmentation with autologous fat work?

The process of breast augmentation begins with gentle liposuction at selected body sites. Excess fat for liposuction is usually available on the abdomen, thighs, or hips. Typically, about 400 - 500ml of fat is extracted. This fat is then processed and purified to ensure the best possible quality.

 

During the breast surgery, the processed autologous fat tissue is precisely injected into all areas of the breast. The volume is increased, and an aesthetically pleasing breast shape and size are sculpted. The advantages of breast augmentation with autologous fat lie in the use of the body's own tissue. In contrast, implants are foreign bodies.

 

The result of autologous fat augmentation looks maximally natural, feels natural, and does not need to be replaced.

 

However, it is important to note:

  • This technique is not suitable for every body

  • Multiple sessions may be required

Breast augmentation and pregnancy

We need to discuss the best timing for your procedure.

 

  • After pregnancy, we recommend waiting 3 months before undergoing breast surgery. This applies to breast augmentation, breast reduction, and breast lift. The reason is that the breast tissue needs time to recover, even if you are not breastfeeding.

  • After weaning of breastfeeding, we recommend a waiting period of 3 - 6 months. Here, too, the focus is on the recovery of breast tissue, but also on avoiding the increased risk of infection after breast feeding.

  • Are you planning to become pregnant? If you are considering a pregnancy, we advise you to  wait with a breast augmentation or breast lift until afterwards. Even though there is no medical reasons against a breast augmentation, we recommend waiting so your beautiful result will last as long as possible.

Can you breastfeed after breast augmentation?

The ability to breastfeed is usually preserved, although this cannot be guaranteed of course.

Our breast augmentations are performed at Operationszentrum Wetzikon, close to Zurich. The operation takes
approximately 1 hour and is usually performed under general anesthesia. You are in the best hands with our very

The operation

Our surgeons have the best tissue-handling skills. The goal is minimal blood loss, as well as fewer hematomas, less
swelling (meaning faster recovery!), and a low risk of complications.

 

  • If the implant is positioned submuscularly - i.e. under the pectoral muscle - the muscle is lifted, and a pocket is
    formed underneath.
    If the implant is positioned above the chest muscle, a pocket is formed between the muscle and breast tissue.

  • Finally, the breast implants are inserted, in the selected size, and precisely placed.

  • To ensure a sterile implant, we use the 'No-Touch' method.

Various incision techniques exist:

  • We prefer a small 3-4 cm incision in the inframammary fold. This access provides the best overview and
    reduces the risk of bleeding or infection. The scar is mostly, hardly visible.

  • An incision at the edge of the areola may be a good alternative. However, this method is only usable for small
    implants.

  • We avoid an incision in the armpit due to the higher complication rate.

After the implants are inserted, we check for the symmetry of the breasts, we rule out active bleeding, and we close
the tissues with 3 layers of sutures. At the end of the operation, a dressing is applied, and very rarely (almost never!) drains are used to drain wound fluid, which are removed after 2-4 days.

What to consider after breast augmentation

We know our patients feel most comfortable at home (and heal best there too!) and you are allowed to go home the same day. Most patients can be picked up after only a few hours. You must not actively participate in traffic on the day of surgery. And once at home, we recommend that the first night you not be alone.

In case of problems in the first few days after the operation

Upon discharge from the clinic, you will receive a telephone number where we can be reached day and night. Even if you only have questions, you are always welcome to call or write to us.

Questions and Answers about Aftercare and Healing Process after Your Breast Augmentation in Zurich

The first 4 - 8 weeks after the operation are crucial for the healing process and require careful follow-up.

Wearing a compression bra

You must wear a compression bra (similar to a sports bra) continuously for the first 6 weeks.

Exceptions: when showering and washing the bra. This supports the breasts, reduces swelling, and promotes scar healing.

How Long Should Physical Exertion be Avoided after Breast Augmentation?

  • In the beginning, physical exertion should be avoided to not disrupt the healing process. This means:

  • From Day 1, all arm movements are allowed as long as they do not cause pain.

  • Light physical activities can be resumed approximately 4 - 5 days after the surgery.

  • Sports and other intense physical activities should be avoided for 4 - 6 weeks.

  • Depending on the profession, work capacity may be limited for 5 - 10 days.

How Long Do You Have Pain After Breast Augmentation?

Most patients report mild to moderate pain, which is adequately treated with painkillers. This should improve quickly after 2 - 3 days. Upon discharge from the clinic, you will be provided with painkillers for the first 5-7 days at home.

How to Sleep After Breast Augmentation?

We advise our patients to sleep on their backs during the first few weeks. We recommend against lying on your stomach for the first 6 months so that the implant pockets can heal undisturbed.

When Can You Shower After Breast Augmentation?

We allow our patients to shower normally the day after the surgery (please do not bathe). The dressings will be changed at the follow-up appointments at the clinic. The first appointment often takes place after 5 - 7 days.

 

During this phase of healing, we happily recommend Zinzino products for personalized nutritional supplementation. Studies have shown a reduced complication rate and improved wound healing.

How Long Does Swelling Last after Breast Augmentation?

Immediately after breast augmentation, the breasts may feel relatively firm to the touch (temporarily!). This is due to the normal swelling after the procedure. After a few weeks, the breasts should feel soft and natural. The foreign body sensation caused by the breast implants tends to decrease rapidly, according to our experience.

When can You start Scar Care?

After 2 - 3 weeks, your incisions no longer need wound dressings or plasters. Now you can start treating your scars. We recommend, for example, Bepanthen scar ointment, twice daily. The scars will be red the first few weeks. Over the course of several months, they will fade. Scar healing and the entire healing process vary individually. You should be prepared for the final result to take up to 6 months. Scar formation disturbances are rare and can often be improved with conservative treatment (silicone patches, microneedling), so that an operative correction is rarely necessary.

How Long Can Numbness Last?

Initially, temporary numbness or hypersensitivity may occur in the nipples. In most cases, nipple sensitivity returns to normal within a year after breast augmentation. As the injured skin nerves heal, temporary "reinnervation pains" may occur. These are sudden, needle-like pains in the nipples and breast. If these pains occur, they indicate that the skin nerves are regrowing and sensitivity will normalize.

Is It Allowed to Fly After Breast Augmentation?

In general, you can fly immediately after breast augmentation.

What risks and potential complications exist with breast augmentation?

As with any surgical procedure, breast augmentation carries potential risks and complications. We provide comprehensive information about these and take the time to answer all your questions. Careful planning, our extensive experience, and state-of-the-art surgical technologies minimize the risk to a minimum.

Swelling and Bruising

In the first days to 2 - 4 weeks after the operation, the breasts will be swollen and rarely there are visible bruises. These usually do not require treatment. However, if there is postoperative bleeding, a repeat operation may be necessary.

Infections

Infections of the breast are very rare and manifest with redness, pain, warmth, and possibly fever, and are treated with antibiotics.

Wound Healing problems

Wound healing problems after breast surgery are rare and it often affects smokers. Healing after surgery relies on an adequate blood flow. Therefore, patients should refrain from smoking starting 2 weeks before surgery until at least 2 weeks afterwards. If necessary, nicotine replacement therapy can be used. In the case of wound healing problems, it may be necessary to re-suture the wound.

Bottoming Out

Smooth-surfaced implants have a possibility of "bottoming out”. This means the inframammary fold cannot hold the implant, and the implant descends 1 - 2 cm. In such cases, corrective surgery is necessary. The implant must be repositioned higher, and the fold reinforced with non-absorbable sutures. This is a rare complication that is relatively easy to correct.

Foreign Body Reactions

The use of silicone implants can lead to foreign body reactions, such as capsular contracture or, in the worst case, lymphoma (this problem has never been diagnosed with smooth-surfaced implants!). The symptoms of capsular contracture usually include hardening and deformation of the breast combined with pain. The exact causes are unknown. Various factors likely play a role, including infections, bleeding, genetic predispositions, and smoking. Capsular contracture is treated with massage, exercises, anti-inflammatory medication, and, as a last resort, surgically.

 

The latest generation smooth-surfaced implants, shows less foreign body reactions of the breast. According to several studies, the capsular contracture rate with Motiva implants is below 1%.

Breast Implant Illness

Breast Implant Illness (BII) refers to nonspecific symptoms that some women with breast implants report. These include fatigue, muscle pain, joint pain, or mental impairments. The exact cause is unclear. Affected women should consult their doctors and consider comprehensive medical analysis.

 

Medical examination before breast surgery can be crucial to minimize potential risks. Gynaecological evaluation may be useful, for example, if there are family members with breast cancer.

Implant Replacement and further options: Long-term Care

The needs and desires of our patients may change over time. Implant replacement or further procedures, such as breast lifts or reductions, are part of our long-term care. Our experienced team is here to support you even after the operation to ensure that you are satisfied with the results.
